About Us
The Medi-Cal Care Coordination Program is offered by the California Department of Health Care Services and administered by APS Healthcare. The program provides select, eligible Medi-Cal members a 24-hour nurse advice line to talk about health concerns, help locating a place to receive regular, ongoing health care, and help finding resources such as food, shelter and transportation in their community. The program also helps members work with their doctor or health care provider to develop an individual health care plan to improve overall health.
The Medi-Cal Care Coordination Program works with physicians, nurses, nurse practitioners, clinics, hospitals and facilities in their local offices to coordinate their member’s care and reinforce the provider’s plan of care for their patients. This partnership ultimately creates a stronger continuum of care, improved health outcomes and cost savings.
