The Medi-Cal Care Coordination Program provides select, eligible Medi-Cal members a 24-hour nurse advice line to talk about health concerns, help locating a place to receive regular, ongoing health care, and help finding resources such as food, shelter and transportation in the community. The program will also help you work with your doctor or health care provider to develop a health care plan to help you feel your very best.

The Medi-Cal Care Coordination Program offers you many benefits. We are here to:
- Answer your questions with a 24-hour Nurse Advice Line
- Help you find somewhere to regularly get health care such as at a clinic, a doctor’s office or a nurse practitioner’s office
- Provide health information and education materials mailed to you and on the program website to help you understand and manage your health conditions
Having A Medical Home Is Important
Having a medical home gives you someone to talk to about your health care and someone who knows your medical history. A medical home is a doctor’s office, clinic or other health care provider that you go to regularly:
- If you are sick or hurt
- If you need preventative care
- For follow-up care to treat chronic health problems
- For a referral to specialty care if needed
A good relationship with your doctor or nurse will help you:
- Make good decisions about your health
- Make sure you get the right immunizations (shots)
- Find help in your community
- Become and stay healthier
