Providers & Stakeholders

The Medi-Cal Care Coordination Program supports providers and assists members in becoming and staying healthy. The program supports members by reinforcing the treatment plans developed by their health care provider and by educating patients on making responsible decisions about their health care.

Medi-Cal Care Coordination Program Helps Providers

  • “Beyond the office”  An extension to providers
    • Assist members to establish and effectively use a medical home
    • Help members with transportation and community resources
    • Help member to keep their health care appointments
    • Coordinate and support  member’s discharge planning and follow-up care 
    • Provide member education to support providers, utilizing evidence-based action plans and encouraging self-management techniques
    • Coordinate with providers to promote member’s adherence to drug regimens that are economical and clinically effective
  • Assist providers to increase the number of patients who receive care consistent with evidence-based practice guidelines
  • Provide on–line information and resources for providers
  • Reduce avoidable inpatient and ER utilization
  • Improve patient centered care coordination with feedback through periodic reporting and clinical updates to providers

Community-Based Model
Community-Based staff placed in hospitals, Federally Qualified Health Centers, Community Health Centers and high-volume Medi-Cal provider offices to help coordinate and reduce barriers to care. By locating community-based staff in local settings the Medi-Cal Care Coordination Program is able to immediately provide members:

  • Post-discharge care coordination
  • Intervention and resource assistance
  • Specially trained nurses for telephonic education/health coaching  between office visits
  • Identification of future risk factors

Client Benefits
All members receive access to a 24-hour Nurse Advice Line, help establishing a medical home, support on how to follow their health care provider’s advice and treatment plans, and assistance in making healthier behavior choices—such as smoking cessation, exercise, proper diet, and stress management. When necessary, Health Coaches also conduct assessments for behavioral health, substance abuse and addiction issues and work with the client to find community resources and develop client specific treatment plans.
