Available Resources and Help in Your Community

The Medi-Cal Care Coordination Program is here to help you find the assistance and resources you need to live a healthier life. We can help you find a doctor, clinic or nurse practitioner who will help you take care of your health on a regular basis. Below is a list of counties with resources containing some clinics and community health care centers accepting Medi-Cal. You may contact clinics yourself or talk to Medi-Cal Care Coordination Program staff about how to schedule an appointment. Staff can also help you find services through a Federally Qualified Health Center (FQHC). All FQHCs accept Medi-Cal insurance. A more extensive provider and clinic directory is available by calling program staff at 1-800-223-1150.
